Participation is voluntary.
Exact residential addresses will never be publicly displayed.
Information may be anonymized and aggregated for analysis.
Participation does not create a physician-patient relationship.
You must be the child's parent or legal guardian, or an adult reporting on your own history, and at least 18 years old.
You may withdraw your data at any time by contacting us with your Household ID.
Your consent is recorded with a timestamp and the exact version of this language.
